Rabies: Why anyone who works with animals should consider an annual vaccination

Rabies is one of the few diseases in veterinary and human medicine that is almost always fatal once symptoms appear, but it is also one of the most preventable. For farmers, farm workers, hunters, veterinarians and anyone else who regularly handles domestic or wild animals, this makes prevention a matter of real, practical importance.
The risk is real, and dogs are not the only threat
Globally, dog bites cause the great majority of human rabies deaths – around 99% of cases worldwide, according to the World Health Organization. But rabies is not confined to dogs. It circulates in a number of wild carnivore species as well, and in southern Africa, jackal are a well-documented reservoir of the disease. Outbreaks of jackal rabies can for example spill over into domestic dogs and into cattle on commercial farms and the farmers and workers will be among those requiring post-exposure treatment after contact with infected jackal or infected livestock.
Anyone working closely with livestock or wildlife on a farm is realistically at higher risk of an undetected or unrecognised exposure than the general public.
Why vaccination still matters even though treatment after a bite works
When it is known that a person has been in contact with a rabid or possibly rabid animal, effective treatment exists and is routinely given. This is called post-exposure prophylaxis (PEP): Thorough wound washing, a course of rabies vaccine and – depending on the exposure – rabies immunoglobulin. When started promptly, PEP is highly effective at preventing the disease.
The problem is that PEP depends entirely on the exposure being recognised in the first place. On a farm, that is not always straightforward:
An animal may not yet be showing obvious signs of rabies when the contact happens, since the virus can be present in saliva before clinical symptoms develop.
Minor scratches, licks on broken skin, or contact during the handling of a sick or injured animal are easy to miss or dismiss at the time.
In more remote areas getting to a clinic quickly, and having immunoglobulin available, is not always guaranteed.
This is exactly where pre-exposure vaccination (getting vaccinated before any known exposure) adds real, additional protection. If someone who works with animals has already had the pre-exposure series, and a rabies exposure later occurs, the follow-up treatment they need is simpler and faster – just two further vaccine doses, with no requirement for immunoglobulin. It also provides a safety margin for exactly the kind of exposure that might otherwise go unnoticed or unreported.
In short: post-exposure treatment remains essential and effective when an exposure is identified. Pre-exposure vaccination doesn't replace that but is an extra layer of protection for the exposures that aren't caught in time.
Who should consider it
International guidance (from the World Health Organization and equivalent bodies) recommends pre-exposure vaccination for people who work directly with animals that could carry rabies, or who are otherwise at ongoing occupational risk. This includes:
Farmers and farm workers who regularly handle cattle and other livestock
Anyone working closely with dogs on the farm
Persons regularly exposed to jackal or other wildlife, including through predator control or hunting
Veterinarians, veterinary technicians, and animal handlers
Once vaccinated, protection can be maintained over several years, with periodic booster doses recommended depending on ongoing risk of exposure.
